Roles and Responsibilities

• Provide quality, personalized healthcare services to clients in their homes.
• Collaborate with the healthcare team to develop and implement patient care plans.
• Monitor and assess patients' conditions, documenting changes and communicating with relevant parties.
• Administer medications and treatments as prescribed.
• Educate patients and their families about health management and self-care strategies.
• Maintain a safe and hygienic environment for patients.
• Participate in continuous education and training to enhance skills and knowledge.

Desired Candidate Profile

• Valid DOH/DHA nursing license.
• Bachelor’s degree in Nursing.
• Strong communication and interpersonal skills.
• Compassionate, empathetic, and dedicated to patient care.
• Ability to work independently and as part of a team.
